Before Oncology Therapy

  • Perform a complete clinical and radiological examination of the patient.
  • Once the dental diagnosis has been established, correct prophylaxisshould be carried out.
  • Cancer treatment results in lowered immunity and any dental infection may become more severe during cancer treatment.
  • The following treatments will be prioritized before starting oncologic treatment:
  1. Caries and pulp infections should be eliminated 15 to 20 days prior to the start of oncological therapy.
  2. Perform a correct prophylaxis, extracting all the relevant dental pieces, leaving at least 21 days of healing, to ensure perfect healing of the tissues.
  3. Perform basic periodontal treatment (oral hygiene and curettage) if necessary.
  4. Eliminate ill-fitting dentures to avoid all possible trauma to the patient’s mucosa.
  5. Make the patient aware of oral hygiene, explaining that it should be done 3 times a day, using pastes with a concentration of 1450ppm of fluoride. Use of dental floss and 0.12% alcohol-free chlorhexidine rinses.

During oncology therapy

  • Perform fluoride applications to minimize the risk of caries.
  • No smoking.
  • Use appropriate rinses, soft toothbrushes, prescribed by the dentist and hygienists to reduce the risk of oral thrush due to the lowered defenses caused by the oncological treatment.
  • Avoid any invasive treatment in the oral cavity. If necessary, make an interconsultation between Oncologist and Dentist.
  • If the oncological treatment was in the oral cavity, dental splints could be made to protect the salivary glands.
  • To go to the dental professional to minimize the consequences of the oncological treatment as much as possible:
    • Xerostomia (decrease in the amount of saliva).
    • Mucositis (Inflammation of the oral mucosa).
    • Any other infection that may appear (fungal, viral or bacterial).

After cancer therapy

  • Re-perform a general clinical and radiological check-up to verify the new oral health status.
  • Prophylaxis and, if necessary, treatment of caries and endodontics.
  • Extractions, dental implants and removable prostheses are contraindicated for at least one year after the end of the oncological treatment and afterwards it will always be necessary to make an interconsultation with the oncologist.
  • In case of dental extraction, if there is no other option, antibiotic coverage will be provided (48 h before / 7-15 days after).
  • Perform check-ups to the patient approximately every 3 months during the first year after the oncological treatment; after that, they will be extended in time if the dental professional considers it necessary.
